Senior Care Roadmap

Taking care of an elderly loved one starts by creating a senior care plan. There are five main elements to this plan:

01 LEGAL

Organizing your loved one's legal affairs is a key part of their senior care plan. Their needs can change quickly, requiring you to move fast and have legal paperwork ready. Otherwise doctors, hospitals, banks, insurance companies and others may very well ignore your wishes. In addition to a will, make sure your loved one has a financial power of attorney and medical power of attorney.

02 MEDICAL

Organizing your loved one's medical information is the next part of their senior care plan. This includes the name and contact info of their doctors, hospitals, medical offices, pharmacies, etc. Additionally, document the name, dosage and purpose of all medications, including supplements.

03 FINANCIAL

It’s important to talk to your loved one about how they manage their finances NOW so you can be prepared to help them in the future. Gaining access to their finance and insurance world is a process that often starts with a difficult conversation.

04 HOUSING

Housing is often a difficult conversation, so talking to your loved one early about their wishes and what's feasible is very important. There are many different options including aging-in-place, moving in with you or another family member, assisted living, independent living communities, and more.

05 CAREGIVER SUPPORT

This is the most often overlooked, but possibly the most important part of your senior care plan: a plan to care for yourself.  Most of us are not prepared for the mental, emotional, spiritual and, yes, even physical toll of caring for an elderly loved one. It's important that you have a personal support plan for yourself so that you don't get burnt out along the way.

We're honored to walk this path with you!

Hi, I'm Cindy. Welcome to Senior Transition Care.

I created Senior Transition Care to give caregivers the resources and support I couldn’t find when I suddenly had to figure out how to take care of my own rapidly aging parents.

My journey was overwhelming, frustrating, lonely—and a plethora of other emotions. But I want yours to be different, and I'm here to help.

The best place to start is by scheduling a consultation call. On this call, I’ll help you get started creating your own senior care plan. It’s totally free. I only get paid if you decide to use any referrals.
